Open-ended questions on an injury form are an essential baseline to getting the full story of what actually happened before, during, and immediately after an injury.  The injured worker should complete this form in their own words and ideally in their own handwriting. The philosophy of ‘see attached’ means that not only should an injury form be filled out thoroughly and completely, as much additional information should be provided such as pictures, videos, equipment manuals, etc. as appropriate.
